We have a number of resources available to ensure every youth makes a seamless transition to his or her community school. A social worker or counselor is assigned to work with the youth while enrolled at Passages Academy. The youth’s social worker helps students, families, and partners understand transcripts, school options and educational pathways. The counselor and social worker helps youth navigate readmission to a community school by setting up school visits with the receiving school and supporting youth through the application process.
Every youth enrolled in Passages Academy has a Plan to Succeed: Individualized Learning Plan. Plan to Succeed NYC is an online platform for students to set goals and track benchmarks towards their success. This interactive tool combines student-driven self assessment, goal setting and monitoring, research, exploration and learning. Students in Passages will have an electronic individual learning plan through Plan to Succeed NYC.
